What is Peer Pathfinders?

The Peer Pathfinders was established to meet the unmeant needs of individuals experiencing homelessness and substance use disorders, who are also homeless or at risk of homelessness or at risk of overdosing.

At CORE Health Support Center our Peer Pathfinder SUD Peers will assist you in navigating and obtaining services to address barriers to independence and recovery. These services include addressing housing, financial resources, transportation, habilitation and rehabilitation services, prevocational and vocational services. Peer Pathfinders intent is to reach those most at risk through Peer Outreach services in Homeless Encampments and Emergency Departments. Peer Pathfinder Eligibility Criteria & Target Population:

  • Adults (age 18 or over)

  • Individuals with Opioid Use Disorders (OUD) at risk of overdose

  • Stimulant Use Disorder (SUD) at risk of overdose

  • Target: Individuals who are suspected of Substance Use and experiencing homelessness or who are at risk of homelessness

  • Target: Individuals experiencing a Substance Use Health Crisis and are utilizing emergency room settings
